FAILURE TO THRIVE

"Failure to thrive? Failure to thrive???" my mom said in disbelief. I, of course, at that time, didn't know what FTT means, although I didn't have a good feeling about it too, seeing mom's reaction when she read Nunu's file after the doctor left. I heard she muttered something like, "Apalah these young doctors nowadays..."

Only after Nunu was discharged (and when I looked at the discharge slip that Nunu was hospitalised for bronchiolitis AND poor weight gain) did I asked mom what the doctor meant by 'failure to thrive'. Momma was careful with her words, and chose the best definition just enough for me to understand. It simply means that Nunu isn't gaining weight well.

I said, "What the..? Didn't they look at her growth chart? She was on the normal growth percentile, what?" Mom just shrugged. It bugged her when she read the report, surely she had a different opinion. So should I be bugged too?

Nunu was 7.2kg when she was admitted. Frankly, I was worried sick because she was 7.5kg just 3 days before that, and 7.8 kg a little over a week before that. Worried that the weight was dropping too fast, but I knew it's gonna go back up once she's back on her feet. But it never occured to me that she's has poor weight gain, considering she was only 2.82kg at birth (as compared to most of the 3kg++ I know) , and it's normal for a baby's weight to fluctuate if he/she falls sick.



This was Nunu on the night she was admitted. Frankly, I didn't have the time to feel all so excessively 'kesian' for her & emotional.

I have mixed feelings about this issue. FTT. As if there's some consolation for me (and mom), there is never an exact or agreeable medical definition for FTT anyway. A quick research revealed to me that FTT is more of a descriptive term rather than a diagnosis. However, most of the sites I visited concluded that FTT means that the child:

- Is at or below the 3rd to 5th percentile for height and weight; or
- Has failed to gain weight as expected, as shown by dropping two growth percentiles
FTT also means that a child is undernourished, which could have been caused by not only organic factors in terms of nutrition but emotional factors as well. Some have underlying medical conditions which contributed to the arrested physical growth.

I understood why mom was aghast at the young doctor for writing 'failure to thrive'. She must have been really offended, a grandchild of a doctor to be called FTT. Not only that it's a wrong diagnosis, the doctors should have tracked her charts, plot them nicely and monitor how she'll do in a couple of weeks. You don't just look at a small & skinny kid & declare them FTT.

What she did RIGHT was asked me a lot of questions, that ranges from what she eats for breakfast (I answered bread & cheese & she gave me an incredulous look!) to how much milk she's taking & how her formula is prepared.

So I had this issue slowly eating my brains out for two whole days - Saturday & Sunday. When the paed came on Monday, he said nothing of Nunu's small stature. He just concentrated on her condition and decidedly said she requires no medication at all for bronchiolitis. We were relieved that it wasn't pneumonia as the on-call doctors suspected earlier. Can you imagine, if someone is sick with pneumonia & you only treat him/her with antibiotics 2 days later??? It's weird here in Malaysia that the government hospital's (read: poor people hospital) pharmacy is closed on weekends.

So, I'm targeting that Nunu should triple her birth weight by the time she reaches 1. That's *taptap calc* 8.4kg, right? 1.2kg in less than a month's time. Will she be able to do it? Even now, I'm still dazed with all sorts of (mis)information & advise people felt free to dispense about her diet.

Don't give defrosted BM or food, nanti sejuk dada. No potatoes, they say, nanti budak penat. No butter, nanti lungs berkahak. No fruits or juices. No nestum or cereal. No chicken or anchovies, nanti batuk. No biscuits too. Habis anak aku nak makan apa???? Takkan nasi & carrot jek???

Well, Nunu's doing very well. She's eating non-stop. She's terrorizing everyone and turning the house upside down (but she's very 'malas' to walk/tatih!). Definitely getting cleverer (dah cerdik sekarang ni, dorang kata) in sooooo many ways, I don't know where to start.
Yeah, she even takes her syrup with a smile these days.

in my opinion nunu looks happy and healthy ... do look at the doctor's charts and make sure there's no miscalculations etc, take it with a pinch of salt.

Looking at it on a more positive light, at least now you are aiming for Nunu's weight gain diet - jangan over sampai hit the other end of the scale dah lah:)

regarding hospitals, in the UK things are not perfect either. did you know that hospitals are only fully functional between 9am-5pm? After 5pm, there are only 4 doctors looking after the WHOLE medical wards, the CT/MRI/Ultrasound departments are closed, pharmacy is closed and everything grinds to a stop. don't even start about weekends - nothing AT ALL happens during the weekend. every patient has to wait till monday before normal service is resumed (but of course, they'll get important antibiotics etc222).

another thing about baby food, when we went back to malaysia last year we couldn't help but notice the limited choice for baby food & supplements. over here there are various types of milks - all with appropriate certifications (and warnings that they could not compare to breast milk), baby foods in jars, tins, bottles, biscuits, snacks, juices - you name it. halal brands are also available.

(not that we buy much of these, they're expensive.)

I almost choked with laughter when I read this. Somehow I wish it was true. Ajab (and soon Ayeed) was brought up on a high carb diet, including potatoes. In fact, one of the very first solids he had was mashed potatoes. Tak penat2 pon budak tu? :D

*sigh ....* All these so-called "advice". In the left ear, out the right ear!

Ya, kids her age have very "bouncy" weight gains and losses - up one month, down another. As long as her weight's within the normal range, then I think it's fine. Even if it's a little low, or high, it's ok too. Not all kids have weights that are straight down the middle of the normal range, kan?

That doc who gave Nunu the FTT diagnosis, did he/she really go through her past history or just concluded simply by looking at her weight charts?

On another note, eeeee .... Nunu nekkid! Better keep Aniq's eyes averted! Haha! :D

if this makes you feel better...aisyah juz went for her 1yr checkup and she's a mere 8.02kg...she eats good food...
(i think. potatoes/chikins/anchovies/etc/etc)..cumer recently her appetite flunctuates and eats only 1/2-3/4 of her usual bowl.

i stopped worrying already because paed (and another paed for second opinion) says as long as she's healthy and active, should be ok. i think aisyah mesti kecik punyer kalau stand by side with your nunu. heheheheheheeh.

Sesak nafas sekejap baca pasal doktor tuh. Bea1 was also hospitalised once for pneumonia. Glad Nunu is ok now;).

mynn, salt, pepper & other perencah also must prepare hor, hehe. I thought hosp. kat M'sia je teruk, so kat UK pun seloow gak ye? One thing, I do believe that the baby food market here is slower coz parents prefer to feed their children home cooked food, including yours truly. The only baby food i bought are nunu's biscuits, & the only cereal she takes (on very rare occasions) are the free samples we got from those trips to clinics! I even get nunu's formula for free these days hehe. And I think that disclaimer, that "breast milk is best", is a huge must on all formula cans.

moby, yg tension nya tht doc just saw her once! nvm, put tht issue behind me oredy. I dont care abt the potato diet anymore, she's lapping everything in her bowl nowadays. I just make sure there's a balanced meal in her daily diet. Problem solved, rite?

ibu aisyah, nolah! i doesnt make me feel better! How small is small? Nunu is 73 cm? Aniq?? Naufal?? How tall & how much do u all weigh?

Mama23beas! 3 beas & soon to be 4!!thnks! it must have been h*** for us mums when our babies are sick. It just makes me feel soooo lucky to have a relatively healthy child, compared to other babies who have much more serious illnesses.

Anyway, hope Nunu will feel very much better now and gain more weight.

Hmm.. maybe all the hospitals in this whole wide world is slow (gov ones). Unless you send the patients in one of those hospitals in the popular tv series, you'll get results before the show ends (say 1 hour?). Heheh...

i'll remind mummy to bring me to the clinic to check my height and weight and i'll report to you. i *think* i've only just barely tripled my birthweight now, though all that running around seems to easily burn up a lot of our energy and nice baby fats! mummy's not too happy, she wants me to stay all round and chubby, just like everyone does (including nunu's doc there, i see). but it's not my fault, i eat like a (baby) horse!

and wooooaaaahhhhhhhhhhh......

Someone's having a birthday very soon (not u lah, Aniq! :P). Let's hope she's not in her birthday suit on that day! Hehe!

Aunty Anne, I think you can rest easy. Little 1yr old me is only 8.1kg now.

I've almast been on the smallish side. Why, Thunder Thighs Aniq looks bigger than me! :o

But my doc says I'm very healthy and developing well so who cares? :p

He did say I have a big head tho.... :p

And to Aniq: Ewah ewah...anak dara orang oi brudder! Mentang-mentang la bakal... :p

aikkss .. looks like I've been eating all the wrong kind of food too..
Don't worry too much Aunty as long as lil nunu is happy, strong and healthy .. gosh she even willingly drinks her syrup like it's fruit juice .. I wish I can be like that

naufal..big head means your brain is growing...sure can fill lotsa stuffs hor. :D but i thot all lil people got big heads...doctors always make us worry.
